The RAI HC is a comprehensive care and service planning tool for adults in home and community based settings and can be used to assess persons with chronic care needs as well as those with post-acute care needs. The instrument is generally used with the frail elderly or persons with disabilities. The focus of the tool is on a person’s level of functioning and quality of life by assessing needs, strengths and preferences. The RAI HC includes Client Assessment Protocols as well as scales for ADLs, cognition, communication, pain, depression and other areas, quality indicators, a case mix system RUG-III/HC, and screening systems to identify appropriate care pathways for clients (i.e., MAPLe).
